WAIVERS AND DISCLOSURES

  • I am legally permitted to work in Canada, and I am not receiving any Social Assistance or Employment Insurance benefits.
  • The Employer/Client will record your working hours on a timesheet. It is YOUR responsibility to sign in and out. Failure may result in loss of pay.
  • I understand I may be subject to a background check. I authorize GTA Work Care to investigate my criminal and employment records.
  • You must follow all company health and safety procedures.

EMPLOYMENT TERMS AND CONDITIONS

  • In case of sickness or emergency, inform your Supervisor and GTA Work Care immediately.
  • Cell phone use during working hours is strictly prohibited.
  • Missing hours must be claimed within 2 weeks.
  • Disrespectful behavior may result in termination.
  • You must follow Government and Employer safety regulations.
  • I authorize GTA Work Care to store my photograph for ID purposes.
